What is medication therapy management?
Medication therapy management (MTM) is an educational service provided by pharmacists focusing on patients with uncontrolled chronic conditions (ex: diabetes or hypertension) supporting healthy behaviors and lifestyle changes for better health outcomes. The pharmacist works closely with the primary care provider and dietician to support patients.
