What forms must I complete?
Please complete all forms before your appointment, including initial enrollment paperwork; general consent, and release request forms; and a sliding fee scale application – with proof of address, proof of income and photo ID – if available.

What should I bring to my appointment?
Patients are also requested to remember to bring a photo ID and insurance cards, if available, as well as a list of all current medications and known allergies. Neither is required to receive services.
Do I need to know my family medical history?
While not required, preparing an up-to-date family medical history will give providers a clearer view of your health and potential health risks. This information will be useful in providing you with the most personalized and effective medical treatment plan possible.
Should I prepare questions for my doctor?
Our Fetter team is your partner in health. If you have questions, we would love to provide answers. We welcome patients to come to their appointments with any questions about their health so that we are able to meet their needs and put their minds at ease.
