All patients are requested to complete the patient registration form, which captures basic personal and contact information and release request.
All patients are also asked to complete the sliding fee discount application.

Complete Registration Paperwork
All new patients receive a patient packet that includes available services and location, general consent, privacy notice and release request forms.

Provide Photo ID
Patients are asked provide photo ID and insurance cards, though it is not required for service. If needed, our team can capture a picture and upload it to your patient chart. All patients are welcome; those with or without insurance.
